This problem was very much noticed on old ford car models.
It sounds like the distributor stator/pickup is defective, this sends an erratic signal to the Ignition control module (called the TFI module) coil and that causes the timing to go wacko. The Ignition control module and the stator are mounted on and inside the distributor change them both, the distributor must come out to replace the stator. There was a recall on may of these trucks, contact a Ford dealer. Another problem is that the ignition switch comes apart, this also was recalled on many models. Below is a diagram of the distributor for your reference.
